  Standardize the DllGetClassObject() prototypes so they match the MSDN
  definition.

- Don't prefix the functions DllCanUnloadNow, DllGetClassObject and
  Dll(Un)RegisterServer with the dll name so that the compiler can check
  the prototypes.
